You will need to get a reputation of Thane with the Orcs in the stronghold in order to be granted access. The reputation is not gained through quests with Orcs in the stronghold and is not affected by your Orc reputation elsewhere. You start with a reputation of Thane of 0 among Orcs, regardless of the reputation of other Orcs. The reputation increases by one for every 25 Orcs in the stronghold.
- Your Orc reputation with those Orcs.
- The threshold of how much Orcs you need to be Blood-Kin with in order to get the Thane title.
- The level of the Stronghold where you are currently leveling. In other words, the greater the number of Orcs in the stronghold, the more likely it is that you'll receive the title. Try to place your stronghold between level 5 and 10, preferably at level 9.
Because of the way the mod is built, the reputation with other Orcs is restricted to how many Orcs in your stronghold (including your own orc blood), and not to your overall orc reputation. For example, if you are good with Orcs, you will still be good with Orcs even if you are not Thane with any Orcs. This means that if you want to become Thane with a specific Orc in the stronghold, you need to be Thane with that Orc first and foremost.
Your level is important, because your level affects your reputation with other Orcs (but not your orc reputation). You can get a Thane title from a stronghold that is at any level higher than you; the level of the stronghold in the mod determines the level from which you start gaining reputation. For example, if you are level 20 and have 10 Orcs in the stronghold, you'll be Thane from level 21 onwards. If you are level 10 and have 20 Orcs in the stronghold, you'll be Thane from level 11 onwards.

For example, if you complete the quest for Narzulbur's old hideout from Mor Khazgur and give it to Narzulbur's shaman, the second time you got the quest item it would give you the quest for Korsh Rufus that the shaman is offering to you. If, before giving the hideout to Narzulbur, you gave it to Jarik, the old Orc leader, the second time you got the quest item it would give you the quest for Jarik that the shaman is offering to you.
